Looking at it I don't think the bullet went through. I think it just pushed the material into the skin. The blood started flowing after the vest was removed because it was plugging the hole. The blood is not from the lung, it's the wrong shade and consistency. If the round had entered there it would have pierced the lung.

I don't think it's fake though.
