Default Any SQL and PHP coders in house? I need some help with something

I'm building a php web form where when items get entered into the form, the items all get inserted into a SQL database for storage and tracking purposes.

The form works. When you submit it, it goes through, the info is emailed over with no issues, and there are no SQL errors that come up.

However, my problem is that the info is not being inserted into the SQL table I set up. I think its something small as far as why its not doing that, but I could use a 2nd set of eyes from someone who knows alot about this stuff.

If there's anyone here who knows php and SQL really well, please shoot me a PM with your email address. I'll send you a zip file with the 2 files and the phpMyAdmin log in to see the tables I set up. This is proprietary info so please only people who I can trust and have been on the site and in the community for awhile, shoot me a message on this.

Thanks for the help!
