I always tip at least a couple of dollars for it. Anybody else do it?

depends on what it is. Almost always $1, sometimes $2 for carryout. The girl complimented my car last night, so I did $2

Pizza delivery, and maybe resteraunts are the only places that it is common to tip in Australia. So no, I don't even tip for carry out, and it isn't expected either.

Is it commonplace to tip for everything in America?

^I work in fine dining and we do to go orders. It doesn't have to be fast to be carry out. I tip on it because I am a kitchen worker, and whether you eat in or take it home, someone in the establishment still has to do the work for you to enjoy your meal.

Most people who wait tables or are a server of any kind get way less than minimum wage. Their pay is based on patrons tipping and tipping correctly. Many establishments tax their workers based not on the tips they claim, but pull taxes out according to the final register tab at the end of the shift. So think twice before stiffing someone. I leave at least 20% in most cases.

generally no. i tip based on what they're doing for you and how good a job they are doing, i don't use percentages. so for caryout, they're not doing much beyond the standard cooking the food. therefore no tip. buffets, reduced tip since all they really do is give you something to drink. sit down restaurant yes for sure and it depends on how well the server does his/her job. i've tipped as low as nothing and as high as 40%. any delivery person usually gets a tip from me since i see that as an extra service.

mostly like this because i worked at restaurants for about 8 years when i was growing up, started when i was 13. began busing tables and washing dishes and worked up to serving the tables and doing the first 2 things most of the time as well. didn't care to get into cooking for some reason.

but here's one interesting thing. i dont usually tip based on the cook's work. thats because almost all the time the cook does not see ANY tips at all - it goes to the serving staff and a smaller cut to the kid who buses the tables. usually the cook has a higher wage to compensate.
